Transdermal estradiol may actually reduce visceral fat accumulation compared to no treatment. The weight gain women notice in midlife is driven by declining estrogen's effect on insulin sensitivity, muscle mass loss, sleep disruption, and shifting fat distribution — not by the hormones used to treat those symptoms. If you start HRT and gain weight, the cause is almost always something else (sleep debt, stress cortisol, calorie creep) worth investigating with a clinician.
