Synthetic progestins (medroxyprogesterone, norethindrone) are more likely to cause bloating, water retention, and mood symptoms than micronized bioidentical progesterone. If you bloat on combined HRT, switching to oral micronized progesterone — taken at bedtime — usually resolves it and adds a mild sedative benefit. Vaginal or rectal progesterone is another option for women who are particularly sensitive. Bloating in the first 4–6 weeks of any new HRT regimen is common and typically self-resolves; persistent bloating warrants a clinician check.
