Different symptoms respond on different timelines. Hot flashes, night sweats, and sleep usually improve within 2–4 weeks of starting transdermal estradiol at an adequate dose. Mood, brain fog, and joint pain often take 6–8 weeks. Vaginal symptoms and libido can take 8–12 weeks, especially if vaginal estrogen is added. If you see no improvement at all by week 6, your dose is most likely too low — this is the most common reason HRT "doesn't work" and is a fast adjustment with your clinician. Full benefit is typically realized by 3 months.
