Ozempic (semaglutide) and Zepbound (tirzepatide) are clinically appropriate for many midlife women with BMI ≥27 with weight-related comorbidities, or BMI ≥30. Phase 3 trial outcomes in postmenopausal subgroups match or exceed results in younger populations. The biggest difference in midlife is that GLP-1s work best when paired with HRT (which restores insulin sensitivity), weight training (to preserve muscle), and adequate protein (≥1 g/lb lean mass). Without those, GLP-1 weight loss includes a higher proportion of lean mass loss, which midlife women cannot afford.
