There is no automatic age at which HRT must be stopped. The 2022 NAMS Position Statement explicitly states that continuation past age 65 is appropriate for many women when the benefits — symptom control, bone protection, quality of life — outweigh risks. The decision is reviewed annually and based on your personal cardiovascular, breast, and clot risk profile, not an arbitrary number. Women who stop abruptly typically have hot flashes return within weeks; tapering over 6–12 months reduces rebound. Vaginal estrogen, separately, can be continued indefinitely.
