As of July 25, 2026, 11 state Medicaid programs cover GLP-1 medications for adult weight loss — down from a peak of 16 in October 2025. Every other state covers these drugs only for a different medically accepted indication. Here is the full picture, the change log, and what to do if your state is not on the list.

What each status means

Covers GLP-1s for weight loss
Medicaid covers an FDA-approved anti-obesity GLP-1 for weight management in adults, almost always with prior authorization.
Covered — sharply restricted
Coverage exists but is limited by high BMI thresholds, step therapy, or bariatric-surgery-aversion criteria.

The state removed adult weight-loss coverage during 2026. Other medically accepted indications remain covered.
Other indications only
Medicaid covers GLP-1s for type 2 diabetes, cardiovascular-risk reduction, obstructive sleep apnea, or MASH — but not for weight loss on its own.

Why the same drug is covered for one diagnosis and denied for another

Federal Medicaid law requires states to cover a drug for its medically accepted indications, but it specifically permits states to exclude “agents when used for anorexia, weight loss, or weight gain” (42 U.S.C. § 1396r-8(d)(2)). That single carve-out — written decades before GLP-1s existed — is the entire reason coverage is a coin flip based on your ZIP code. Diabetes coverage is effectively mandatory. Obesity coverage is optional, and optional benefits are the first thing cut when a state budget tightens.

“Required” coverage is still subject to each state’s prior-authorization and medical-necessity rules. EPSDT requires coverage of medically necessary care for members under 21 even where the adult benefit is excluded.

Losing a benefit is not the same as losing the treatment. Three pathways usually remain open, and the first one is the most commonly missed: a qualifying diagnosis you already have. Why does Medicaid cover Ozempic but not Wegovy for weight loss?

Federal law requires Medicaid to cover a drug for its medically accepted indications but specifically allows states to exclude agents used for weight loss (42 U.S.C. section 1396r-8(d)(2)). Diabetes is not an excludable use, so Ozempic and Mounjaro are covered nationwide for type 2 diabetes. Weight loss is excludable, so obesity coverage for Wegovy, Zepbound, and Saxenda is optional and varies by state.

Does menopause change GLP-1 eligibility?

Menopause is not itself a covered indication, but the metabolic shift of perimenopause and postmenopause commonly produces the conditions that are covered — insulin resistance progressing to type 2 diabetes, rising cardiovascular risk, and worsening sleep apnea. A clinician who evaluates the full metabolic picture may identify a qualifying diagnosis that a weight-focused visit misses.

Sources and methodology

We anchor the national count to KFF, then read the primary state document — Preferred Drug List, prior authorization form, or provider bulletin — for every covering state, every recently changed state, and every high-restriction state. A state is counted only when its own source supports coverage for obesity or weight management. Diabetes-only, cardiovascular, sleep-apnea, MASH, and EPSDT-only pathways are not counted as adult weight-loss coverage. Where we could not reach a primary source, we rely on KFF’s published count and say so.
