Estradiol cream is a topical hormone preparation containing bioidentical estradiol suspended in a dermatologic base. Because 503A compounded cream is not an FDA-approved drug product, individual formulations are not required to demonstrate bioequivalence to commercial products in randomized trials. Serum estradiol monitoring is therefore a routine part of dose titration.
