Mechanism of action

Dual agonist of the GLP-1 and GIP receptors. The GIP arm enhances insulin secretion and appears to amplify GLP-1's effects on satiety and energy expenditure beyond what a GLP-1 alone achieves.

How does tirzepatide compare to semaglutide?

Head-to-head in SURPASS-2 (type 2 diabetes), tirzepatide produced greater weight loss and A1c reduction than semaglutide 1.0 mg at every dose. In weight-management trials, average loss is roughly 1.5x higher.

How is tirzepatide dosed?

Once-weekly subcutaneous injection. Titrated from 2.5 mg upward every 4 weeks to a maintenance dose (5, 10, or 15 mg/wk). The slow titration is critical for GI tolerability.

What are the side effects?

Same class effects as semaglutide — nausea, constipation, diarrhea, reflux. Generally well tolerated when titrated slowly. Black-box warning for thyroid C-cell tumors (rodent data).

Is compounded tirzepatide legal?

Compounded tirzepatide can be prepared by licensed 503A pharmacies when there is a valid prescription and the FDA shortage list applies. The shortage was resolved in late 2024, narrowing compounded access.

How long does it take to work?

Appetite suppression typically starts within the first week. Meaningful weight loss appears by week 8–12; the curve continues to drop through ~72 weeks before plateauing.
