Peptide evidence grades

A single scale applied to a field that usually avoids one.

Short answer: Grade A means multiple randomized placebo-controlled human trials with consistent results — semaglutide, tirzepatide, and tesamorelin qualify. Grade B is a single adequate randomized trial or consistent smaller ones. Grade C is preliminary human data. Grade D is animal or mechanistic data only, which describes most of the peptide market.

What each grade means

  • A — multiple randomized placebo-controlled trials, consistent direction, adequate sample.
  • B — one adequate randomized trial, or consistent small trials with a plausible effect size.
  • C — open-label or observational human data only.
  • D — animal, in vitro, or mechanistic data; no meaningful human evidence.

Why mechanism is not evidence

Almost every failed drug in history had a compelling mechanism. Torcetrapib raised HDL and increased mortality. A receptor-binding story tells you a compound does something; only a controlled trial tells you whether that something helps.

Why do vendors cite animal studies?

Because for most of the compounds they sell, that is all that exists. Rodent healing data is genuine science — it just does not establish human benefit, dose, or safety.

Does grade D mean it does not work?

No. It means nobody knows. That is a meaningfully different claim from "proven", and it should change the price you are willing to pay and the risk you accept.
