Yes — headaches are a known early HRT side effect, usually settling within 4–8 weeks. Oral estrogen causes more headaches than transdermal because once-daily oral dosing creates hormone peaks and troughs that can trigger migraine-prone brains. Women with a history of menstrual migraine often do better on a steady-delivery patch or gel rather than pills. If headaches persist, options include lowering the dose, switching to transdermal, or trialing a continuous (vs cyclic) regimen so estrogen levels stay flat. Sudden severe headache, especially with visual changes, requires immediate evaluation.
