SMS Opt-In Terms & Consent

Program Description

Kindr Health operates a transactional and conversational SMS program for patients and prospective patients of our menopause and hormone-replacement-therapy telehealth service. By opting in, you agree to receive text messages from Kindr Health at the mobile number you provide. Messages may include:

• Appointment reminders and scheduling confirmations
• Prescription and shipping notifications
• Care-team replies to questions you submit
• Account, billing, and security alerts
• Educational follow-ups related to your treatment plan

How Patients Opt In

Opt-in is collected via a clearly-labeled checkbox on our public web forms. The checkbox is unchecked by default and only appears once a user has entered a mobile number. The full consent language displayed at the point of opt-in reads:

"I agree to receive SMS text messages from kindr Health at the mobile number provided, including appointment reminders, care updates, and account notifications. Message frequency varies. Message and data rates may apply. Reply HELP for help, STOP to unsubscribe. Consent is not a condition of purchase. See our Privacy Policy and SMS Terms."

Opt-in is collected on the following public forms:

• /contact — general patient inquiries
• /refer — refer-a-friend program
• Patient intake (during clinical onboarding)

Mobile opt-in data and consent are never shared with third parties or affiliates for marketing purposes.

Supported carriers include AT&T, Verizon Wireless, T-Mobile, Sprint, Boost, U.S. Cellular, MetroPCS, and others. Carriers are not liable for delayed or undelivered messages.

Privacy

Your mobile information, including phone number and consent record, is handled in accordance with our Privacy Policy and HIPAA Notice. We do not sell, rent, or share mobile opt-in data with third parties for marketing.
