What is Hormone Replacement Therapy?

Hormone replacement therapy — HRT — restores the estrogen, progesterone, and sometimes testosterone that the body produces less of during menopause. When these hormones decline, symptoms follow rapidly and often severely: hot flashes, night sweats, sleep disruption, brain fog, mood changes, low libido, and vaginal dryness.

HRT is the most clinically effective treatment for menopausal symptoms and has been supported by decades of research. The North American Menopause Society, ACOG, and the Endocrine Society all recommend HRT as the first-line treatment for symptomatic menopause in healthy women under 60 or within 10 years of menopause onset.

The science

Why hormones matter so much.

Common questions

Is HRT safe?

For most healthy women under 60 or within 10 years of menopause onset, HRT is safe and effective. The Women's Health Initiative findings that previously cast doubt on HRT have been substantially re-analyzed and reinterpreted by NAMS, ACOG, and the Endocrine Society. Your Kindr provider conducts a full personal and family medical history review and individualizes risk-benefit assessment — HRT is never prescribed as a one-size-fits-all decision.

How is compounded HRT different from commercial HRT?

Commercial HRT comes in fixed doses and fixed combinations. Compounded HRT is prepared by a state-licensed, FDA-registered pharmacy to the exact dose, ratio, and delivery method your provider prescribes. This allows protocols that match your specific symptom profile and biology. Compounded medications are not FDA-approved drug products.

What if I had breast cancer — can I still use HRT?

Personal history of estrogen-receptor-positive breast cancer is generally a contraindication to systemic HRT. Your Kindr provider will review your oncology history in detail and discuss non-hormonal options such as SSRIs, SNRIs, gabapentin, or local non-estrogen vaginal therapies that may safely improve symptoms. Always coordinate with your oncology team.

How long before HRT works?

Most women notice improvement in hot flashes and sleep within 4–6 weeks of starting an appropriate dose. Mood, libido, and cognitive symptoms often take 8–12 weeks to fully respond. Vaginal symptoms may improve in 2–4 weeks with local estrogen. Dose optimization typically continues for 3 months until your provider confirms the right protocol for you.

Can I use HRT if I still have a period?

Yes. Many women in perimenopause benefit from HRT while still cycling. The protocol — particularly the progesterone component — is designed differently for perimenopausal women than for postmenopausal women. Your Kindr provider will tailor your regimen to your cycle pattern and symptoms.

What happens if I stop HRT?

Symptoms typically return when HRT is discontinued — particularly hot flashes and sleep disruption. Stopping is a personal decision made with your provider. If you do choose to stop, your provider can taper you down gradually to minimize rebound symptoms. There is no medical requirement to stop HRT at a specific age.

Is testosterone safe for women?

Yes — at the low physiologic doses appropriate for women. Testosterone is one of the most undertreated hormones in menopausal women. The Endocrine Society endorses low-dose testosterone for hypoactive sexual desire disorder in postmenopausal women.
