Breast tenderness is one of the most common HRT side effects, particularly in the first 4–8 weeks. It happens because estrogen stimulates breast tissue — similar to premenstrual breast tenderness. Most cases are mild and resolve as your body adjusts. If tenderness persists beyond two months, kindr providers often lower the estrogen dose, switch from oral to transdermal, or adjust the progesterone. New persistent breast tenderness, a new lump, skin changes, or nipple discharge always warrants clinical evaluation regardless of HRT.
