Oral estradiol is the original modern HRT. It is effective for hot flashes, sleep, mood, and bone protection. Its trade-off is metabolic: passing through the liver raises clot and stroke risk slightly compared with the patch. For some women, the pill is still the right choice. For others — particularly with cardiovascular risk factors — the patch is safer.

Side effects

Nausea is more common than with the patch (take with food). Breast tenderness, bloating, and mild headaches in the first few weeks are typical and usually settle. Less common but more serious: clotting symptoms (leg swelling, calf pain, sudden chest pain or shortness of breath) require immediate medical attention.

Pairing with progesterone

If you have a uterus, oral estradiol must be combined with a progestogen — micronized progesterone (Prometrium) 100-200 mg at bedtime is the most common pairing.

Is the pill less effective than the patch?

Equally effective for symptom control. The difference is in the safety and metabolic profile, not efficacy.

Can I switch from pill to patch?

Yes. Many women switch when cardiovascular risk factors appear or as they age. The transition is straightforward.

Does the pill cause weight gain?

Trial evidence does not support significant weight gain attributable to oral estradiol. Bloating in the first weeks is common.

Can I take the pill with morning coffee?

Yes. Take with food if nausea is an issue.

How quickly does the pill start working?

Hot flashes improve in 2-4 weeks; sleep and mood within 4-8 weeks.
