HRT often improves libido indirectly — by relieving vaginal dryness that makes sex painful, restoring sleep, lifting mood, and easing fatigue. Estrogen alone rarely directly raises sex drive in postmenopausal women. The hormone most directly linked to libido in women is testosterone, which also falls in midlife. Low-dose physiologic testosterone for women with persistent low libido despite optimized estrogen is supported by international clinical guidelines (Global Consensus 2019), and is a routine part of the kindr provider toolkit when appropriate.
