Compounded Bioidentical Hormones

Compounded bioidentical hormones are custom hormone preparations made by a 503A compounding pharmacy to a clinician’s prescription. The active ingredients (estradiol, progesterone, testosterone, DHEA) are bioidentical — the same molecules used in FDA-approved bioidentical hormone therapy — but they are prepared at custom strengths, in custom combinations, or in delivery vehicles not available commercially. They are appropriate for specific clinical situations; they are not automatically better than FDA-approved bioidenticals.
Two separate concepts get conflated. "Bioidentical" describes the molecule — it has the same chemical structure as the hormone your body produces. "Compounded" describes the manufacturing — it was prepared to a custom prescription rather than mass-manufactured. You can have FDA-approved bioidenticals (Estrace, Prometrium) and compounded bioidenticals (custom estradiol-progesterone cream). 503A pharmacies compound for individual patient prescriptions; 503B "outsourcing facilities" can compound in larger batches for office use. Both are regulated, but 503B facilities operate under stricter FDA cGMP requirements. Most compounded bioidentical HRT is dispensed by 503A pharmacies. Verify the pharmacy is state-licensed and ideally PCAB-accredited.
A properly compounded bioidentical preparation from a reputable 503A pharmacy is safe. The risks come from poor pharmacy practices (potency variability, contamination) and from supraphysiologic dosing (especially with pellets). USP <795> governs non-sterile compounding (creams, capsules); USP <797> governs sterile compounding (injectables). Ask your pharmacy about USP compliance and potency testing.
Compounded bioidenticals are rarely covered by insurance. Expect $60–$120/mo for a single hormone, $100–$200/mo for combinations. If insurance coverage matters, FDA-approved bioidenticals (which are usually covered) should be the first option.
