Vaginal dryness and GSM: the symptom no one wants to talk about

Published April 15, 2026 · Last updated May 10, 2026

Genitourinary syndrome of menopause is common, progressive, and highly treatable.

GSM affects more than half of postmenopausal women and worsens over time without treatment. Symptoms include vaginal dryness, painful intercourse, urinary urgency, and recurrent UTIs.

What works

  • Low-dose vaginal estrogen — extremely safe and highly effective for most patients, including many breast cancer survivors after oncology coordination
  • Vaginal hyaluronic acid suppositories
  • Ospemifene (a SERM) for those who cannot use vaginal estrogen
  • Regular sexual activity (with or without a partner) preserves tissue elasticity

GSM rarely improves on its own. Earlier treatment is easier and more effective than waiting.
