The short answer: menopause is not just a reproductive event. It is a metabolic one. Within a few years of the final menstrual period, most women experience measurable shifts in insulin sensitivity, body composition, lipid profile, and mitochondrial efficiency — even without any change in diet or activity. What is metabolic menopause?

When to test — and what to test

Fasting insulin, HbA1c, a full lipid panel with ApoB, and a waist circumference measurement give a complete metabolic snapshot. Fasting glucose alone is not sensitive enough — insulin resistance often shows up in fasting insulin years before glucose rises. Any woman in perimenopause or the first decade postmenopause should have these markers every 12 months.
