For most women, transdermal estradiol (patch or gel) is preferred over oral estradiol. Skipping the liver first-pass means lower risk of venous thromboembolism, no increase in C-reactive protein or triglycerides, fewer migraine triggers, and steady serum levels instead of daily peaks and troughs. Oral estradiol is still reasonable for women without cardiovascular risk factors who prefer pills. The patch is changed once or twice weekly depending on brand (Climara is weekly; Vivelle-Dot and Minivelle are twice weekly).
