Bioidentical hormone therapy uses hormones that are molecularly identical to those produced by the body — estradiol and progesterone — rather than synthetic alternatives. It is the form of HRT most widely prescribed by menopause specialists today. Available to women across Arkansas's humid subtropical climate — entirely online.

Where you live in Arkansas should not decide whether you get treated. Most of Arkansas is federally designated as medically underserved, and menopause care in the state is concentrated around UAMS in Little Rock and the Northwest Arkansas metro. With around ~390k of Arkansas women in the midlife band and specialists clustered in Little Rock, Fayetteville, Fort Smith, the drive is often the reason care never starts.
Every clinician who treats you holds a full, active Arkansas license in good standing with the Arkansas State Medical Board, and prescribes only within the standards that board sets — a documented history, a real evaluation, and a written record for each prescription. Any pharmacy dispensing to a Arkansas address is licensed with the Arkansas State Board of Pharmacy. We do not bill Arkansas Medicaid (ARHOME) or commercial Arkansas plans; care is a flat monthly subscription, and we will provide an itemised receipt if you want to submit it yourself.
Arkansas sits in South Central with a humid subtropical climate, and that shows up in how symptoms present here. Hot, humid summers and limited specialist density make telehealth menopause care especially valuable.

Bioidentical hormone therapy uses estradiol and progesterone that are chemically identical to the hormones your ovaries produced before menopause. This is a molecular definition — not a regulatory or marketing category. Many FDA-approved products (estradiol patches, gels, and oral micronized progesterone) are bioidentical, as are compounded preparations from 503A pharmacies.
The distinction that matters clinically is between bioidentical hormones (estradiol, progesterone) and synthetic analogs (conjugated equine estrogens, medroxyprogesterone acetate). Bioidentical formulations have a different receptor-binding and metabolic profile and are the current first-line choice in menopause specialty practice.
Bioidentical estradiol binds estrogen receptors alpha and beta with the same affinity as endogenous estradiol, restoring physiologic signaling in the brain, bone, cardiovascular system, and urogenital tissue. Micronized progesterone activates the progesterone receptor identically to ovarian progesterone and, through its metabolite allopregnanolone, exerts calming effects on the central nervous system via GABA-A receptors.
Because the molecules are identical to what the body already recognizes, they are metabolized through the same pathways as endogenous hormones, without the estrogenic and androgenic metabolites produced by some synthetic progestins.
The 2022 NAMS Position Statement, the Endocrine Society clinical practice guideline (Stuenkel et al., 2015), and the International Menopause Society Global Consensus all recognize bioidentical estradiol and micronized progesterone as evidence-based, first-line hormone therapy for symptomatic women within 10 years of menopause onset or under age 60.
The KEEPS and ELITE randomized trials used bioidentical transdermal estradiol and micronized progesterone and reported favorable cardiovascular and cognitive outcomes when treatment was initiated in this early-menopause window. The E3N cohort found a substantially better breast-cancer risk profile with estradiol plus micronized progesterone than with synthetic combinations.
Most side effects are mild and settle within the first 4-8 weeks. Report anything severe or persistent to your prescriber.
