The distinction that matters clinically is between bioidentical hormones (estradiol, progesterone) and synthetic analogs (conjugated equine estrogens, medroxyprogesterone acetate). Bioidentical formulations have a different receptor-binding and metabolic profile and are the current first-line choice in menopause specialty practice.
Bioidentical estradiol binds estrogen receptors alpha and beta with the same affinity as endogenous estradiol, restoring physiologic signaling in the brain, bone, cardiovascular system, and urogenital tissue. Micronized progesterone activates the progesterone receptor identically to ovarian progesterone and, through its metabolite allopregnanolone, exerts calming effects on the central nervous system via GABA-A receptors.
Because the molecules are identical to what the body already recognizes, they are metabolized through the same pathways as endogenous hormones, without the estrogenic and androgenic metabolites produced by some synthetic progestins.
The 2022 NAMS Position Statement, the Endocrine Society clinical practice guideline (Stuenkel et al., 2015), and the International Menopause Society Global Consensus all recognize bioidentical estradiol and micronized progesterone as evidence-based, first-line hormone therapy for symptomatic women within 10 years of menopause onset or under age 60.
The KEEPS and ELITE randomized trials used bioidentical transdermal estradiol and micronized progesterone and reported favorable cardiovascular and cognitive outcomes when treatment was initiated in this early-menopause window. The E3N cohort found a substantially better breast-cancer risk profile with estradiol plus micronized progesterone than with synthetic combinations.
Most side effects are mild and settle within the first 4-8 weeks. Report anything severe or persistent to your prescriber.
