Compounded HRT

What it is

Compounded HRT is hormone therapy custom-prepared by a state-licensed, FDA-registered compounding pharmacy following USP <795> (non-sterile) and USP <797> (sterile) standards. It is prepared to a specific patient prescription — not mass-manufactured.

Compounded preparations are NOT FDA-approved finished drug products. The FDA regulates the pharmacy and the standards; it does not approve each compounded formulation. This is a meaningful distinction patients should understand.

Common reasons for compounding: a dose not available in commercial form, a delivery method not commercially available (cream rather than patch), or an allergy to an excipient in the commercial product.

How it works

Compounded HRT contains bioidentical 17β-estradiol, micronized progesterone, and/or low-dose testosterone — the same molecules the body produces. The active ingredient is identical to FDA-approved bioidentical HRT; the formulation is custom.

Mechanism of action is the same as FDA-approved bioidentical HRT: estradiol acts on estrogen receptors, progesterone protects the endometrium and provides sleep/mood benefit, testosterone supports libido and well-being.

Who it's for

• Patients whose clinical needs are not met by available commercial doses
• Patients who need a delivery method (e.g., cream) not available commercially
• Patients with allergies to excipients in commercial products
• Patients whose providers determine compounded is the appropriate option

Who should not take it

• Patients for whom FDA-approved bioidentical HRT meets clinical needs (it should be the default)
• Active hormone-sensitive cancer
• Active liver disease
• Unexplained vaginal bleeding
• Recent blood clot, stroke, or heart attack

Common side effects

• Same as bioidentical HRT generally: breast tenderness, mild bloating, headaches in early weeks
• Skin irritation at application site for transdermal compounded preparations
• Spotting in the first 3-6 months (combined therapy)
