What it is

Vaginal estrogen is low-dose estradiol or estriol applied directly to vaginal tissue. Available as a cream, vaginal tablet, vaginal ring, or vaginal insert.

It is fundamentally different from systemic HRT — the dose is much smaller and systemic absorption is minimal. Serum estradiol levels typically remain in the postmenopausal range during treatment.

It is the gold-standard treatment for Genitourinary Syndrome of Menopause (GSM) — the cluster of symptoms including vaginal dryness, painful intercourse, urinary urgency, and recurrent UTIs caused by estrogen loss in vaginal and urinary tissue.

How it works

Local estrogen restores vaginal epithelial thickness, glycogen production, lactobacilli colonization, vaginal pH, and tissue elasticity. These changes reverse the structural cause of GSM rather than masking the symptom.

Improvement in vaginal symptoms typically begins within 2-3 weeks. Urinary symptoms (urgency, recurrent UTIs) often respond within 8-12 weeks.
