Best 'estrogen supplement' for menopause: honest guide to phytoestrogens

'Natural estrogen supplements' are one of the most-searched terms in menopause — and one of the most oversold categories. Nothing OTC replaces HRT. A few phytoestrogens have real trial data for symptom relief.

Phytoestrogens with real evidence

  • Whole soy foods — best-studied, safest phytoestrogen source
  • Genistein 30-60 mg standardized isoflavone extracts
  • Red clover 40-80 mg standardized isoflavones
  • Black cohosh 40 mg/day — for hot flashes specifically

What has no reliable evidence

Wild yam cream (not converted to progesterone in humans), maca (small benefit for libido, not estrogen), evening primrose oil (essentially no effect on hot flashes).

When to consider HRT

Severe hot flashes, sleep disruption, cognitive fog, or vaginal atrophy that don't respond to supplements within 8-12 weeks are best addressed with prescription HRT. See how HRT works.

Frequently asked questions

What is the best estrogen supplement?

Whole soy foods (edamame, tofu, tempeh) provide the strongest evidence-backed phytoestrogens. Genistein and daidzein modestly improve hot flashes over 12 weeks. Effect is smaller than HRT.

Does black cohosh work?

Mixed evidence. Some RCTs show 20-30% reduction in hot flashes; others show no effect. Standardized extracts (Remifemin 40 mg/day) have the best data. Not for anyone with liver disease.

What about red clover?

Contains isoflavones similar to soy. Modest effect on hot flashes in some trials. 40-80 mg standardized extract daily.

Are DIM and I3C estrogen supplements?

No — they modulate estrogen metabolism (favoring 2-hydroxy over 16-hydroxy pathways). Useful for estrogen-dominant symptoms in perimenopause; not a substitute when estrogen is truly low.

What raises estrogen naturally?

You cannot meaningfully raise blood estradiol with supplements. Weight (fat tissue produces small amounts), phytoestrogens (bind receptors weakly), and reducing gut beta-glucuronidase activity can shift estrogen exposure marginally.
