Bioidentical hormone replacement: what it is, what it treats, and what the evidence actually says.

Bioidentical hormone replacement uses hormones chemically identical to the ones the ovaries produce during the reproductive years. Here is what the evidence actually says — separated from marketing.

The short answer

Bioidentical hormone replacement (BHRT) uses hormones — 17β-estradiol, micronized progesterone, and sometimes low-dose testosterone — that are structurally identical to the hormones a healthy premenopausal ovary produces. Many of the most-prescribed HRT products in the United States are already bioidentical: Estrace, Climara, Vivelle-Dot, Divigel, Estrogel, and Prometrium are all FDA-approved bioidentical formulations.

The term "bioidentical" is often used loosely — sometimes to describe compounded custom preparations, sometimes to describe FDA-approved products, and sometimes as a marketing label. Kindr uses the term the way clinicians do: bioidentical describes the molecule, not how it is made.

Bioidentical vs synthetic vs compounded — the three most confused terms

  1. Bioidentical: chemically identical to human hormones. 17β-estradiol is bioidentical. Micronized progesterone is bioidentical. Both come in FDA-approved formulations.
  2. Non-bioidentical (synthetic): a similar but not identical molecule. Conjugated equine estrogens (Premarin), medroxyprogesterone acetate (Provera), and norethindrone acetate are examples. They act on the same receptors but with different downstream effects.
  3. Compounded: refers to how the medication is prepared — mixed to order by a compounding pharmacy rather than manufactured as a finished product. Compounded HRT can be bioidentical or non-bioidentical.

Most patients who ask for "bioidentical hormones" are best served by an FDA-approved bioidentical product (transdermal estradiol plus micronized progesterone). Compounded is reserved for cases where a commercial dose or delivery form is not available.

What bioidentical hormone replacement treats

  • Hot flashes and night sweats (75–80% reduction within 8–12 weeks per NAMS 2022)
  • Sleep disruption driven by nocturnal vasomotor episodes
  • Vaginal dryness, painful intercourse, and other genitourinary symptoms of menopause

  • Joint aches and skin changes associated with estrogen decline

The evidence — 2026 view

  1. NAMS 2022 Hormone Therapy Position Statement: for healthy women under 60 or within 10 years of menopause onset, the benefits of HRT for symptom relief and bone protection outweigh the risks. Transdermal estradiol is preferred over oral for patients with elevated cardiovascular or clot risk.
  2. The Women's Health Initiative (WHI) revisited: the increased breast cancer signal was seen with conjugated equine estrogens + medroxyprogesterone in women who started HRT more than 10 years after menopause. It does not translate directly to modern bioidentical HRT started at the time of menopause.
  3. Route matters more than molecule for VTE risk: oral estrogen (any type) modestly increases blood-clot risk; transdermal estradiol does not appear to above baseline in observational and randomized data (E3N cohort, ESHRE/IMS 2016).
  4. Progesterone vs progestins: observational data (E3N cohort) suggest bioidentical micronized progesterone may have a more favorable breast profile than synthetic progestins. Randomized long-term data are still developing.
