Dry eyes in menopause: the underrecognized symptom
Burning, gritty, watery, blurry — dry eye syndrome affects up to 60% of postmenopausal women and often goes undiagnosed. The lacrimal and meibomian glands are hormone-sensitive, and when estrogen and androgens fall in menopause, tear production and quality drop with them.
The mechanism
Meibomian glands (which produce the oil layer of tears) and lacrimal glands (which produce the aqueous layer) express estrogen and androgen receptors. Androgen decline is especially important — androgens are the dominant hormonal signal for meibomian gland function.
Treatment ladder
- Preservative-free artificial tears 4–6×/day
- Warm compresses + lid hygiene for meibomian gland dysfunction
- Omega-3s 2–4g/day (EPA/DHA)
- Prescription: cyclosporine (Restasis) or lifitegrast (Xiidra)
- Punctal plugs to reduce tear drainage
- Consider transdermal HRT if not contraindicated (systemic benefits, neutral ocular)
When to rule out Sjögren's
Dry eyes plus dry mouth, joint pain, or fatigue warrants autoantibody testing (SSA, SSB). Sjögren syndrome is a common autoimmune mimic of menopausal dry eye.

Can menopause cause dry eyes?
Yes. Dry eye syndrome prevalence roughly doubles after menopause due to hormonal effects on the lacrimal and meibomian (oil) glands.
Does HRT help dry eyes?
Systemic HRT has mixed evidence — oral estrogens may worsen dry eye, while transdermal is neutral or beneficial. Topical androgens (compounded) show promise.
What is meibomian gland dysfunction?
The oil layer of the tear film breaks down, causing tears to evaporate faster. It is the dominant driver of dry eye in menopause and responds to warm compresses and omega-3s.
Are lubricating drops enough?
Preservative-free artificial tears help mild cases. Moderate-severe cases need cyclosporine (Restasis), lifitegrast (Xiidra), or scleral lenses.
