Estradiol cream: uses, dosing, and how it differs from other estrogens
Estradiol cream can be prescribed two very different ways: as a low-dose vaginal preparation targeting only local tissue, or as a systemic transdermal cream that treats hot flashes and night sweats. The molecule is identical — the dose, application site, and clinical effect are not.
Two products, one molecule
Estradiol cream at pharmacy strength is 0.01% and dosed in grams — 1 g contains 0.1 mg estradiol. Systemic compounded estradiol cream is stronger and delivers 1–2 mg per gram, matched to plasma-level targets.
Vaginal use (GSM)
Applied intravaginally with an applicator. Restores vaginal epithelium, blood flow, and lactobacilli. Systemic estradiol absorption at maintenance doses is negligible.
Systemic use (hot flashes / HRT)
Applied to inner arm or thigh, rotated to avoid skin buildup. Achieves plasma estradiol of 40–100 pg/mL, comparable to a 0.05 mg patch. Preferred for women who prefer to titrate their dose day-to-day.
Combination with progesterone
Any woman with an intact uterus using systemic estradiol — cream, patch, or pill — must also take progesterone (usually oral micronized 100 mg nightly) to prevent endometrial overgrowth. Vaginal-only dosing at maintenance levels does not require progesterone in most women, but discuss with your clinician.

Estradiol cream vs Estrace cream?
Estrace is a brand name of estradiol vaginal cream. Generic estradiol cream is bioidentical to Estrace at a lower cost.
Where do you apply systemic estradiol cream?
On the inner arm, thigh, or lower abdomen. Rotate sites and avoid the breasts.
How much estradiol cream should I use?
Vaginal: 0.5–1 g nightly for 2 weeks, then 2–3 nights per week. Systemic transdermal: 1–2 mg estradiol daily, dose adjusted based on symptoms and blood levels.
Does estradiol cream cause weight gain?
Low-dose vaginal: no. Systemic doses: some fluid retention in first weeks; long-term weight-neutral in most trials.
Is estradiol cream safer than pills?
Transdermal estradiol (cream, patch, gel) bypasses first-pass liver metabolism and does not raise clotting risk the way oral estradiol does. It is now the preferred route for most women starting HRT.
