Ozempic for Menopause

Ozempic is the diabetes brand of semaglutide — the same molecule as Wegovy, at lower doses. It is not FDA-approved for weight loss, but clinicians prescribe it off-label for weight management, especially when Wegovy is in shortage or insurance excludes weight-loss indications. Here is what menopausal women should know.
Semaglutide mimics GLP-1, a gut hormone that signals satiety to the hypothalamus and slows gastric emptying. In menopause, estrogen withdrawal disinhibits the same appetite circuits that GLP-1 normally suppresses — which is why "I am hungry all the time now" is one of the most common perimenopause complaints. Semaglutide directly restores that brake.

If you have type 2 diabetes, insurance will usually cover Ozempic. If you have obesity (BMI ≥30) or overweight + a comorbidity (BMI ≥27 + hypertension, sleep apnea, dyslipidemia), Wegovy is the on-label choice and increasingly covered. Compounded semaglutide remains available through 503A pharmacies during documented shortages or for individualized dosing.
Off-label, yes — many menopause clinics prescribe semaglutide for visceral fat and insulin resistance even without a diabetes diagnosis. Coverage will likely be out-of-pocket or via compounded equivalent.
STEP trial averages: ~10% at 6 months, ~15% at 12 months. Individual results vary; women on HRT plus resistance training tend to do better than diet-and-cardio alone.
Injectable estradiol, patches, and gels are unaffected. Oral estradiol absorption may be slightly delayed by slower gastric emptying — your clinician can switch you to a patch if needed.
