Zepbound for Menopause

Zepbound is Eli Lilly's tirzepatide, FDA-approved in November 2023 specifically for chronic weight management. Unlike semaglutide (GLP-1 only), tirzepatide also activates the GIP receptor — and in the SURPASS-2 head-to-head, it produced superior weight loss. For menopausal women dealing with visceral fat and insulin resistance, that dual mechanism matters.
GIP (glucose-dependent insulinotropic polypeptide) receptors are highly expressed in adipose tissue. Activating GIP enhances insulin sensitivity in fat cells and promotes preferential loss of visceral (intra-abdominal) fat — exactly the fat depot that grows during menopause. Semaglutide does not do this. In SURMOUNT-1, tirzepatide produced ~50% reductions in visceral adipose tissue measured by DXA.
| Month | Dose |
|---|---|
| 1 | 2.5 mg weekly |
| 2 | 5 mg weekly |
| 3 | 7.5 mg weekly |
| 4 | 10 mg weekly |
| 5+ | 12.5 or 15 mg weekly (maintenance) |
Zepbound retail is ~$1,060/month. With commercial insurance covering weight-loss medications, copays range $25-$100. Eli Lilly's LillyDirect program offers single-dose vials direct to self-pay patients: 2.5mg ~$349/mo, 5mg ~$549/mo — meaningfully cheaper than the auto-injector pens.
Zepbound has produced greater average weight loss in trials. Wegovy has a longer real-world safety record. If both are accessible to you, Zepbound is the higher-efficacy starting point — but switching between them mid-course is routine.
Yes. Same guidance as semaglutide — transdermal estradiol is preferred over oral if you are on a GLP-1, but the patch is the safer HRT route anyway.
Rapid weight loss of any kind can trigger telogen effluvium (temporary diffuse hair shedding) 2-4 months in. It resolves with adequate protein and stabilization. Menopausal estrogen decline can compound this — HRT often helps.
