HRT after hysterectomy: estrogen-only guide

If your uterus has been removed, endometrial protection is unnecessary in most cases. Estrogen-only therapy — the arm of the WHI that actually reduced breast cancer incidence — is the standard approach.

Estrogen-only regimens

  • Estradiol patch 0.025–0.1mg/day
  • Estradiol gel/spray
  • Oral estradiol 1–2mg if no clot risk

Frequently asked questions

Do I need progesterone after hysterectomy?

Usually no. Exceptions: prior endometriosis or ovarian remnant syndrome.

What did WHI show for estrogen-only?

In the CEE-alone arm (hysterectomy cohort), breast cancer incidence was lower than placebo.

How long can I stay on it?

No arbitrary stop date. Duration is a shared decision based on ongoing benefit.
