HRT and blood pressure: what the evidence actually shows
Whether HRT raises your blood pressure depends almost entirely on route. Oral estrogens pass through the liver and can activate the renin-angiotensin system. Transdermal estradiol (patch, gel, spray) bypasses first-pass metabolism and is BP-neutral in most women — and mildly lowering in some.
Why route matters
Oral estrogens are absorbed through the gut and delivered to the liver at high concentration, where they increase angiotensinogen production. This can raise blood pressure in salt-sensitive women. Transdermal estradiol bypasses the liver entirely and does not have this effect.
The clinical decision
- Normal BP: any route reasonable, transdermal preferred for cardiometabolic safety
- Hypertension controlled: transdermal estradiol + micronized progesterone
- Hypertension uncontrolled: treat BP first, then transdermal HRT
- History of hypertension in pregnancy or oral contraceptives: transdermal only

Does HRT raise blood pressure?
Oral estrogens can raise BP in some women via hepatic first-pass activation of angiotensinogen. Transdermal estradiol does not raise BP and may lower it modestly.
Can I take HRT if I have hypertension?
Yes, with transdermal estradiol as the preferred route. Uncontrolled BP >160/100 should be addressed first.
Does progesterone affect BP?
Micronized progesterone (Prometrium) is BP-neutral or mildly lowering via aldosterone antagonism. Synthetic progestins are less favorable.
How often should I check BP on HRT?
Baseline before starting, at 3 months, then annually. More often if you have pre-existing hypertension.
What if my BP goes up on HRT?
Switch from oral to transdermal, verify progesterone type, and address contributors (alcohol, sodium, sleep apnea). Rarely a reason to stop HRT.
