HRT and uterine fibroids: what to know

Uterine fibroids typically shrink after menopause as estrogen falls. HRT can stimulate residual fibroids, but most women with fibroids can still use HRT safely with the right monitoring plan.

Practical protocol

  • Baseline pelvic ultrasound
  • Transdermal estradiol, lowest effective dose
  • Continuous combined regimen preferred
  • Repeat ultrasound at 6–12 months if symptomatic

Frequently asked questions

Does HRT make fibroids grow?

It can — especially with higher-dose oral estrogen. Transdermal at the lowest effective dose is preferred.

Can I still use HRT with fibroids?

Usually yes, with baseline ultrasound and periodic follow-up if symptomatic.

Which route is best?

Transdermal estradiol + progesterone or Mirena IUD.

What symptoms warrant reassessment?

New heavy bleeding, pelvic pressure, rapid uterine enlargement.
