The HRT window of opportunity: why timing matters
The 'window of opportunity' isn't marketing — it's one of the most durable findings in menopause research. Women who start HRT within 10 years of their final period (or before age 60) get cardiovascular protection. Women who start after that window see neutral or slightly increased cardiovascular events. The mechanism is atherosclerosis timing.
Why atherosclerosis timing matters
Estrogen is vasoprotective when arteries are healthy — it maintains endothelial function, prevents plaque formation, and reduces inflammation. But estrogen introduced onto already-established plaque can destabilize it, increasing rupture risk. The window closes as atherosclerosis progresses.
Practical implications
- If you are within 10 years of menopause: start HRT with confidence if indicated
- If you are further out but healthy with no atherosclerosis: individualized decision
- If you have known coronary disease: avoid new initiation of systemic HRT; vaginal estrogen remains safe
What Kindr does
We take a cardiovascular history, calculate ASCVD risk, and decide on HRT accordingly. Older women aren't automatically excluded — but the workup is more thorough.
Frequently asked questions
What is the HRT window?
Within 10 years of the final menstrual period, or before age 60. Starting HRT in this window is associated with reduced cardiovascular events.
What happens if I start later?
HRT is still useful for symptoms and GSM. Cardiovascular benefit is neutral or slightly negative — but not a hard stop.
Why does timing matter?
Estrogen on healthy arteries is protective. Estrogen on plaque-laden arteries can destabilize plaque. Younger vessels haven't developed atherosclerosis yet.
Can I still start HRT at 65 or 70?
Yes, if the indication justifies it (severe symptoms, GSM, bone protection). Use transdermal and lowest effective dose.
How was the window discovered?
Reanalysis of WHI data by age subgroup, plus the ELITE trial and observational cohorts. Consistent signal across multiple lines of evidence.
