What it is

Premarin contains a mixture of conjugated estrogens (primarily estrone sulfate and equilin) extracted from pregnant mare urine.

Oral tablet, also available as a vaginal cream (see Premarin Vaginal).

How it works

Conjugated estrogens are absorbed orally and processed by the liver, then circulate to estrogen receptors throughout the body.

Who it's for

Women with severe vasomotor symptoms when bioidentical estradiol is not available or tolerated
Patients with longstanding successful use of Premarin who prefer to continue

Who should not take it

Estrogen-sensitive cancer history
Elevated clot/stroke/cardiovascular risk (transdermal is generally safer)
Active liver disease

Common side effects

Nausea, breast tenderness, headache early on
Higher VTE risk than transdermal estradiol
Spotting on combined therapy

Evidence summary

FDA-approved for vasomotor symptoms, GSM, and osteoporosis prevention.
WHI 2002 used oral CEE; modern guidelines now favor transdermal bioidentical estradiol for most patients (NAMS 2022).

Brand-name vs compounded

Premarin is an FDA-approved brand-name product. FAQ

Is Premarin bioidentical?

No — it is a mixture of estrogens not naturally produced by the human body (including equilin from horses).

Why do some doctors still prescribe it?

Long clinical history and patient familiarity. For new HRT starts, most modern menopause specialists prefer transdermal estradiol.
