Premarin generic: is there one, and what to take instead.
Premarin is one of the oldest continuously prescribed medications in the United States. Despite that, there is no FDA-approved generic. Here is why, what your alternatives are, and how modern menopause practice thinks about the switch.
The short answer
There is no FDA-approved generic Premarin. Premarin's active ingredient is conjugated equine estrogens — a mixture of at least 10 distinct estrogen compounds isolated from pregnant mare urine. The FDA requires any generic to demonstrate the same complex composition, and after several failed applications (most notably Duramed's attempt in the 1990s and Barr Laboratories' in the early 2000s), no manufacturer has succeeded. Premarin has been Pfizer-owned since 2009 and remains brand-only.
Bioidentical alternatives that are generically available
- Generic oral estradiol (Estrace generic): 0.5 mg, 1 mg, 2 mg tablets. Roughly $10–$30/month.
- Generic estradiol transdermal patch: 0.025, 0.0375, 0.05, 0.075, 0.1 mg/day. Roughly $15–$50/month. Preferred by NAMS 2022 over oral in most patients.
- Generic estradiol vaginal cream: for genitourinary symptoms only, not systemic HRT.
- Estradiol gel and spray: available as brand (EstroGel, Elestrin, Evamist) — no generic yet, but often covered by insurance.

A rough clinical conversion used when transitioning off Premarin:
- Premarin 0.3 mg oral ≈ 0.5 mg oral estradiol ≈ 0.025 mg/day estradiol patch
- Premarin 0.625 mg oral ≈ 1 mg oral estradiol ≈ 0.05 mg/day estradiol patch
- Premarin 0.9 mg oral ≈ 1.5 mg oral estradiol ≈ 0.075 mg/day estradiol patch
- Premarin 1.25 mg oral ≈ 2 mg oral estradiol ≈ 0.1 mg/day estradiol patch

Why modern menopause practice generally prefers estradiol
- Bioidentical to what the ovary produces.
- Available transdermally, avoiding first-pass liver metabolism.
- Extensively studied in KEEPS, ELITE, and the E3N cohort.
- Lower monthly cost with insurance.
Premarin remains a reasonable choice for patients who have done well on it for years — but for new HRT starts, most menopause specialists lead with transdermal estradiol.
