Premature ovarian insufficiency (POI): diagnosis and treatment
Premature ovarian insufficiency (POI) — sometimes called premature menopause — affects 1% of women before age 40 and up to 5% before age 45. It is often missed for years. Without HRT, women with POI face significantly higher cardiovascular disease, osteoporosis, dementia, and mortality risk. This is not optional treatment — it is essential replacement therapy.
Why this diagnosis matters
Estrogen deficiency starting in your 20s or 30s accumulates 15–25 years of extra cardiovascular and skeletal risk before natural menopause would have occurred. Untreated POI raises all-cause mortality by 60–80%. HRT reverses most of this excess risk.
Treatment approach
- Physiologic-dose transdermal estradiol (0.075–0.1 mg patch typical)
- Cyclic or continuous micronized progesterone based on preference
- Combined oral contraceptives an acceptable alternative for younger women
- Continue at least until age 51 — the age of natural menopause
- Bone density baseline and every 2 years
- Screen for co-occurring autoimmune conditions (thyroid, adrenal, celiac)
