Tesamorelin: the FDA-approved visceral fat peptide
Tesamorelin is the only GHRH analog with FDA approval — originally for HIV-associated visceral fat accumulation. In clinical trials, it reduced deep abdominal fat by 15–20% over 6 months. That's why it's increasingly discussed for postmenopausal belly fat.
Why the visceral fat angle matters in menopause
After menopause, subcutaneous fat proportion drops and visceral fat proportion rises — even at stable body weight. Visceral fat drives insulin resistance, cardiovascular risk, and inflammatory aging. A therapy that selectively reduces visceral fat has a different value than one that just lowers the scale number.
The pivotal trials
Two phase-3 trials (Falutz et al., NEJM 2007) enrolled 816 patients with HIV lipodystrophy. After 26 weeks of 2 mg daily tesamorelin, VAT decreased by ~18% vs a 2% increase in placebo. Triglycerides fell 50 mg/dL. IGF-1 rose into the upper-normal range.
How it's used off-label in longevity practice
Physician-supervised protocols typically use 1–2 mg subcutaneous daily for 12–26 weeks, with IGF-1 monitored monthly. Effects on visceral fat plateau around month 6.

Tesamorelin vs sermorelin?

Does tesamorelin cause weight loss on the scale?
Not much. It preferentially removes visceral fat and preserves lean mass, so total scale weight often changes very little — but waist circumference and metabolic markers improve.
Is tesamorelin safe for postmenopausal women?
