Vaginal estrogen cream: the safest, most effective GSM treatment

Vaginal estrogen cream is the single most effective treatment for genitourinary syndrome of menopause (GSM) — the umbrella term for vaginal dryness, painful sex, and recurrent UTIs after menopause. Because absorption is local, systemic exposure is negligible, and even most breast cancer survivors can safely use it under oncology guidance.

Why estrogen fixes GSM

The vagina, urethra, and bladder trigone are all estrogen-dependent tissue. When estradiol falls in menopause, the vaginal epithelium thins, glycogen-fermenting lactobacilli disappear, pH rises, and blood flow drops. That produces the entire symptom cluster: dryness, painful sex, urinary urgency, and recurrent UTIs.

Why the FDA "black box" is being reconsidered

Vaginal estrogen carries the same class-wide FDA boxed warning as systemic estrogen — which experts now agree is inappropriate given the near-absence of systemic absorption. In 2023, four medical societies (ACOG, NAMS/The Menopause Society, ISSWSH, SGO) petitioned the FDA to remove the boxed warning for low-dose vaginal estrogen.
