The short answer
Climara Pro is a once-weekly transdermal patch that releases 0.045 mg of estradiol and 0.015 mg of levonorgestrel per day. Its current label approves it, in women with a uterus, for moderate-to-severe hot flashes due to menopause and for prevention of postmenopausal osteoporosis. Because it already contains a progestin, it is a different regimen from estradiol-only patches such as Climara, Vivelle-Dot, Dotti, or Minivelle and is not interchangeable with them.
Boxed warning on the current Climara Pro label
The current Climara Pro label (DailyMed, March 2026) carries a boxed warning for cardiovascular disorders, probable dementia, breast cancer, and endometrial cancer, based on the Women's Health Initiative estrogen-plus-progestin study. It states that estrogen plus progestin should not be used to prevent heart disease or dementia. In November 2025 the FDA asked hormone therapy manufacturers to remove some boxed-warning language, but a product's warning changes only when its own label is revised.
Contraindications on the label include undiagnosed abnormal genital bleeding, current or past breast cancer, estrogen-dependent cancer, current or past blood clots, current or past stroke or heart attack, liver disease, known blood-clotting disorders, serious allergic reaction to the product, and pregnancy.

Estradiol-only patches need a separate progestogen for people with a uterus. Climara Pro builds the progestin in. That changes who it is for, its boxed warning, and its side effects. People without a uterus are generally prescribed estrogen alone.

Applying the patch
The label directs application to the lower abdomen, rotating sites and avoiding the breasts and the waistline. A new patch is applied every seven days. Used patches should be folded and discarded out of reach of children and pets.

Monitoring
Abnormal or persistent bleeding should be evaluated. The label also warns about blood pressure, triglycerides, gallbladder disease, and thyroid hormone needs, and recommends periodic reassessment of whether treatment is still needed.
